Do It Online

From the 1st of April 2016 Detailed Coded Records Access (DCRA) will be available to request online which requires a username and password to make this request. Please enquire with our medical reception staff if you require a username and password.

Following your online request it is necessary for you to complete a written DCRA request form. Photographic identification is also required to process the request.
